1. What types of documents can I request online?
You can request various documents such
as:
- Transcript of Records
- Diploma
- Certificate of Graduation
- Good Moral Certificate
- Certification of Enrollment
- Other relevant academic documents
2. How do I request a document online? To request a document:
- Visit our college’s official online document request portal.
- Fill in the required details, including your student number, name, and the document(s) you are
requesting.
- Upload any required identification or proof of request.
- Submit the form.
3. Is there a fee for requesting documents?
- All documents are FREE TO CHARGE BUT LIMIT TO NUMBER OF REQUEST.
4. How long does it take to process my request? Processing time varies depending
on the document:
- Transcript of Records: 5-7 working days
Diploma: depends on the availability
Other certifications: 1-3 business days
5. How will I receive the requested document? You can choose between two
delivery options:
- Pick-up: You will be notified when your document is ready, and you can pick it up at the
Registrar’s
Office. Bring two (2) documentary stamps (for TOR requests).
- Digital Copy: A scanned copy will be sent to your registered email address. Please note that
only
certain documents can be delivered digitally.
6. What should I do if I made a mistake in my request?
- If you need
to correct any information after
submitting your request, please contact the Registrar's Office as soon as possible via
registrar@clb.edu.ph or call telephone number (043) 784-7771. Include your request reference
number
to expedite the correction process.
7. Can I track the status of my document request?
- Yes, once your request is submitted, you will
receive a confirmation email with a reference number.
- You can track the status of your request
using
this number through our online portal.
8. Can I request documents for someone else?
- If you are requesting on behalf of another student, you
must provide a signed authorization letter from the student along with a valid ID for both the
student and the requester.
9. What if I encounter technical difficulties while submitting my
request?
- If you experience any
issues with the online request portal, you can contact our IT support team via it@clb.edu.ph or
call
telephone number (043) 784-7771.
- Make sure to provide details of the issue for faster
resolution.
10. What if my document isn’t available after the processing time?

If your document is not ready
within the estimated processing time, you can reach out to the Registrar’s Office to inquire
about
the delay.
